Tri-Town Health Department Community Health Worker/Health Agent The Tri-Town Health Department is seeking a qualified part time (19 hours/week) Community Health Worker/Health Agent to assist our department with community education, outreach, and inspectional services with specific target populations. The CHW will work closely with medical providers, businesses, community at large, restaurants, and other agencies to improve public health and COVID-19 safety guidelines and be trained to conduct sanitary inspections. Requirements: Two to five years' experience in a community setting. Associates Degree required. CHW Certificate within 2 years or at discretion of manager preferred. MA Driver's License with reliable transportation for travel to and from work and within the community. This is a tobacco free position.
